STOP: If you are an ACT Institute graduate registering for continuing education go directly to Returning Student Registration forms.

Application is by eligibility category for corresponding programs designed specifically for each group. The application forms are in PDF format and require Acrobat Reader. If you do not have Acrobat download it from here.

New applicants must complete the following steps:

1. Elgibility
Select the application and Registration forms for your eligibility category from below.

2. Application
Download, print and complete the Application Form specifying the course (s) of training and submission requirements requested.

3. Registration
Download, print and complete the Registration Form specifying the course(s) of training.

4. Training
Application and Registration forms must be received a minimum of six weeks prior to the start date of the selected training program.

5. Send
Application, Submission Requirements and Registration Forms to:

6. Verification
ACT will verify receipt and notify applicants of registration.
